Physical Exam and Immunization Requirements:

· All incoming 12th graders must have a second MCV (Meningococcal Conjugate) vaccine before the start of school

· All incoming 11th graders must have a physical exam submitted before the start of school

Medication Orders and Chronic Condition Action Plans:

· If your child requires daily or as needed medication to be administered by the school nurse, please have your child’s physician complete a Medication Dispensing Form. Medications and the completed Medication Dispensing Form must be delivered to the Nurse’s Office by a parent or guardian.

· If your child is diagnosed with Diabetes, Seizures, Asthma, or Life-Threatening Allergies, please submit an updated action plan (treatment and/or medication orders) to the nurse’s office.

· If your child has been diagnosed with any medical condition that requires special care or accommodation in the school setting, please submit an updated note from their physician to the nurse’s office.
